What is Darling Downs HealthPathways
The Darling Downs HealthPathways Program is led by the Darling Downs Hospital and Health Service in partnership with the Darling Downs and West Moreton Primary Health Network. It seeks to foster a dynamic collaboration between its hospitals, community services spanning across public and private systems and primary healthcare services. The collaborative working partnership increases the collective’s capacity to provide the right quality of care in the right place by the right person at the right time.
Darling
Downs HealthPathways is a web-based portal with information on referral and
management pathways helping clinicians to navigate patients through the complex
primary, community and acute health care system in the Darling Downs region.
Darling Downs HealthPathways is designed to be used at point of care by the primary care network, including general practice, allied health services, community health and community pharmacy.
Who’s involved?
Darling Downs HealthPathways is governed by the Boards of both the Darling Downs Hospital and Health Service and Darling Downs and West Moreton Primary Health Network.
The Steering Committee guides the strategic direction; the Darling Downs HealthPathways team led by the Clinical Director Dr Daniel Halliday who directs and drives the program of work and is supported by the following:
- General Practice Clinical Lead;
- Change Champion
- Clinical Editors;
- Darling Downs HealthPathways Team; and
- Professional Advisory Network.
